Everett Premier Dentistry is pleased to offer an In-Office Membership Program to patients currently not covered by a dental benefit plan.  We want to help you prioritize your oral health, and this program allows for a complete continuation of preventative care.

This is NOT an insurance plan but a membership plan for our loyal patients!

• No monthly premiums -- simply pay the annual fee and get ready to save
• No yearly maximums -- visit the dentist as much or as little as you need to
• No complicated forms to fill out -- just sign up, make the payment, and that’s it
• You can join at any time of year
• Even includes discounts on certain cosmetic procedures which most insurances don’t cover
• No need to worry about figuring out who is in-network or out-of-network
• The whole family can become a member

What Is Covered With The Plan?

• Comprehensive Exam (Initial Exam):  100%
• Periodontal Exam (2 per year): 100%
• Oral Cancer Screenings (2 per year): 100%
• Fluoride (2 per year): 100%
• Limited Oral Exam/Emergency (1 per year): 100%
• Intra-oral Periapicals: 100%
• Bitewing (1 set per year): 100%
• Adult Cleaning (2 or 3 per year depending on plan): 100 %
• Deep cleaning: 15% off
• Sealants: 15% off
• Teeth whitening: 15% off
• Fillings: 15%
• Crowns: 15%
• Dentures and partials: 15% off
• Extractions: 15% off
• Root canals: 15%
• Night guards: 15%
• Implants: 15% off

Terms & Conditions

• Membership is for one year beginning on the enrollment date.
• Membership dues are payable in full upon enrollment.
• Cash, check or credit card payments accepted.
• Annual membership fees may be adjusted annually.
• Member will be informed of any increase in annual membership fee prior to renewal.
• Members are responsible for notifying Everett Premier Dental of any address or contact changes.
• Missed appointment fees are ineligible for the membership discount and will disqualify you for the year. If you miss a cleaning without a 48 hour notice then you’ll forfeit the cleaning for that year.
• Membership may be terminated at will by written notice from the patient to Everett Premier Dentistry.
• Everett Premier Dentistry may terminate the membership by written notice to the patient when terminating the doctor patient relationship or discontinuing the membership program.
• Membership benefits are not transferable, have no cash value and may not be redeemed for cash.
• This is not an insurance plan and is not subject to regulation by the state department of insurance.
• Plan membership cannot be combined with current dental insurance plans.
• No insurance claim will be filed for Members under this plan.
• The plan is for individual use only.  It is not a group benefit plan.
• Payments for additional dental services are the member’s responsibility.
• Additional dental service fee will be based upon the fee at the date of service.
• Payment is due on the date of service to qualify for the discounts detailed above.
• Membership must be current to receive the discount.
• Fees for dental services may change at any time.
• Everett Premier Dentistry retains the right to interpret the program stipulations.
• It is the member’s responsibility to schedule and keep all appointments offered as part of the Dental Membership Program.
• Please notify our office at least 48 hours in advance if you must change a scheduled appointment.
• No deductibles, no pre-authorizations, no yearly maximums, no waiting period.
• No exclusions for elective care.
